CareDocs update (v4.8.5) including essential NHS Assured Supplier requirements

The next update to the CareDocs system, v4.8.5 is now ready to download. This latest software release introduces essential NHS Assured Supplier requirements, including new forms, improved existing forms and new reports to support the improved Emergency Pack.

Emergency Pack Form Additions

Several new forms have been introduced that will form part of the improvements made to the Emergency Pack. These new forms have been ratified to ensure compliance with crucial NHS Assurance standards and NICE guidance NG27.

New Transfer Property List Form

This release includes the introduction of the new Transfer Property List form which can also be used as a standalone form (outside its use within the Emergency Pack), for example, when a Service User is transferred to a new setting. The form includes pre-defined data capture options including:

  • Jewellery.
  • Valuables.
  • Personal aids.
  • Toiletries.

All of which allow the user to complete the record during an emergency hospital admission quickly. And any information captured will be transferred to the Emergency Admission Pack.

New Transfer Form and SBAR Assessment Form

This form is a comprehensive form a provider must complete when a decision needs to be made to admit a service user to the hospital. The form is split into sections with some data already populated as content from the Service Users care files, such as assessments and care plans to improve the efficiency of completing the form. The form is split up into four sections which cover:

  • General information regarding the care category and funding streams in place.
  • Clinical assessments (including allergies, health concerns and diagnosed conditions).
  • Vital observations.
  • Situation Background Assessment Recommendations (SBAR).

It consists of 4 standardised prompt questions to ensure that care staff share concise, relevant, and focused information relating to the Service User, thereby promoting better outcomes in care for each Service User.

Any captured information on this form will be transferred to the Emergency Admission Pack.

New Red Bag Pre-Admission Checklist Form

When a Service User is required to go to the hospital the Red Bag Hospital Pre-Admission Checklist form must be completed. The checklist will prompt the user to ensure all supporting documents that will form part of the Emergency Pack have been gathered.

Any captured information on this form will be transferred to the Emergency Admission Pack.

New Red Bag Post-Discharge Checklist Form

This form enables users to complete a checklist upon the Service Users discharge from the hospital — this forms part of the end-to-end transfer of care between the care setting and the hospital. Note that this form will not be automatically pulled into the Emergency Pack but will act as a standalone checklist within the CareDocs Forms and Reports.

Emergency Pack Updates & Improvements

This is Me Form Improvements

The This is Me form is a comprehensive form a provider should complete when a Service User is admitting to the Care Setting after the initial assessment of care. This form can also be used as a standalone (outside of its uses in the Emergency Pack), for example, where a Service User is transferred to a new setting.

The improved ‘This is Me’ form will support clinicians, nursing, and carers in Health & Social Care settings to understand better who the Service User is when there is a need for support in unfamiliar surroundings. This can reduce a Service Users distress and fears if the professional has information about their needs, abilities and wishes.

The following improvements have been made:

  1. Retitled the form to ‘This is Me.’
  2. Included the form in the Emergency Pack.
  3. Increase data capture to have a more holistic approach to care needs.
  4. Increased auto population from specific care file records related to the Service User.

Emergency Pack Content & Layout Improvements

Improvements to the content and layout of the Emergency Pack have been made, however, the function of producing an Emergency Pack has not changed. This improvement was made to align the CareDocs system with the requirements for NHSX. The following content and layout will be created within the Emergency Pack:

  1. Emergency Pack Front Sheet.
  2. Personal Information.
  3. Emergency Contacts.
  4. Transfer Form & SBAR Assessment.
  5. NEWS2.
  6. This is Me.
  7. Active Medication.
  8. Advanced Care Plan / End of Life.
  9. Transfer Property List.
  10. Red Bag Pre-admission Checklist.
  11. Red Bag Admission Checklist.

This improvement will help support users in staying on top of care records across the Service Users file.
