What are Care Plans?
A Care Plan is an essential and personal guide to caring for a resident. An effective Care Plan will be unique, person-centred and complete, catering for all aspects of day-to-day living. If we think of the Assessment as an opportunity for residents to use their voice, the Care Plan is the care provider’s chance to ensure those voices are listened to by giving a rewarding and fulfilling service. Because of the resident’s involvement in building the plan, maximum benefit is gained from their stay with you.
How are Care Plans used?
CareDocs’ Care Plans are generated automatically using the answers provided in the Care Assessments, and with your input, are tailored to each individual. Developed with the knowledge of care experts and a range of care home staff, our tried and tested Care Plans provide a person-centred care service for all aspects of day-to-day living, to give both residents and staff the best care experience.
Depending on the capacity of your residents, the Care Plan will either address them in first or third person. Each section is fully editable, allowing you to further personalise the information. We also include a risk rating and an indication of the level of assistance required, which can be amended if necessary.
With regular notifications to remind you to update your Care Plans, you can be confident the care you’re giving is always up to date and fully compliant with CQC requirements and other major regulators. Any manual changes to Care Plans are logged within the system, so you can keep track of developments and who made them.

What’s involved in creating a Care Plan?
It couldn’t be simpler to create a Care Plan. It only takes three easy steps!
- Complete the Assessment
- Generate a draft Care Plan
- Finalise the Care Plan
We don’t take shortcuts either. In the Care Assessment phase, we encourage you to include as much information about your residents as possible. And the questions we ask you are based on the answers you provided previously, so you can be sure the final result is bespoke to each individual. The more personalised Care Plans are, the better service you residents will receive.
Some resident information from other parts of the system, such as height, weight, date of birth and certain Body Map and Chart data, works in sync with the Care Plan. We want to make the process as easy as possible for everyone involved so you don’t have to ask or enter the same information twice.
CareDocs was built with the resident at its heart. Completing our comprehensive Care Plan is the final stage in giving a voice to the people who require care and cannot necessarily speak for themselves.
I use Care Plans for specific needs not included in the list
If you find that you could benefit from additional Care Plan sections that aren’t mentioned in the above list, that’s no problem. CareDocs is a highly customisable software which gives you the tools to create your own Care Plan sections.
